Job Description
Wider Circle is seeking a Bilingual Community Health Worker to connect neighbors for better health, improve access to services, and empower members to lead healthier lives. The CHW will be a trusted community member, liaising with health, social services, and members, and assisting with programs and events. The role involves on-site visits and phone/video calls.
Requirements
- Certified Community Health Worker
- Enhanced Care Management experience
- 3+ years of relevant community outreach experience
- Excellent communication and organization skills
- Positive attitude and strong computer skills
