Caregiver

Willow Valley Communities is seeking extraordinary PRN (as needed) Caregivers to join our extraordinary Connections at Home team!

Requirements

• Arrive at each assignment on time and complete the tasks outlined in the plan of care developed for each client.
• Communicate to supervisor on a regular basis regarding the effectiveness/appropriateness of the services being provided or any changes or concerns regarding a client’s health.
• Use good judgment in decision-making and respect confidential matters relating to a client.
• Positively represent Connections at Home
• Current Driver's License required and access to vehicle required
• Must be 18 yrs of age or older
