Requirements
- Conduct holistic, whole-family assessments and work with families to identify their strengths and challenges.
- Act as a Lead Practitioner for Early Help Assessments and Plans
- Facilitate responsive group programmes
- Respond to enquiries and direct contact from various sources, undertaking timely and effective support and regular reviews of family needs.
- Support families in accessing resources, particularly those who have faced barriers to engagement
- Offer practical help and encouragement to parents to develop their confidence and parenting skills.
- Advocate on behalf of babies, children and young people
- Contribute to the delivery of integrated services
- Maintain accurate records in line with data protection regulations and assist in collecting data to monitor progress.
- Build and maintain effective professional links, especially with schools and health partners.
- Attend multi-disciplinary meetings to ensure access to services is timely, appropriate, and well-monitored.
- Act appropriately on any child protection concerns in line with policy and procedures
