Job Description
The Care Coach role supports patients using Durable Medical Equipment (DME) across all product lines, providing patient education, adherence monitoring, documentation, and coordination with internal teams. This involves counseling patients on technology, gathering data, and ensuring compliance with regulations and standards. The Lead role focuses on driving operational excellence, mentoring team members, and escalating issues.
Requirements
- Provide one-on-one mentoring and coaching to patients on device use.
- Educate patients on proper use, benefits, and maintenance of devices.
- Monitor patient adherence and usage data via device software.
- Accurately document all patient interactions and device data in the EMR system.
- Collaborate with internal teams and external partners to resolve patient needs.
- Complete required compliance training and continuing education programs.
