Integrated Care Coordinator

The Integrated Care Coordinator works collaboratively with the Behavioral Health Consultant, providers, and the Integrated Behavioral Health team. They assist in identifying patients who would benefit from behavioral health intervention, gather and provide resources, and perform administrative tasks.

Requirements

• Bachelor’s degree in social service field or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience
• High school diploma or GED from an accredited institution
• Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients
• Excellent customer service skills
• Excellent computer skills
• Knowledge of Arizona Department of Health Services and RBHA requirements
• Knowledge of chronic disease management
• Ability to complete PRAPARE assessments
• Comprehensive and working knowledge of community resources
• Thorough knowledge of current versions of DSM and ICD
• Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices
• Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross
• Bilingual English/Spanish strongly preferred
• Valid Arizona driver’s license
• Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety
