Care Manager - Brooklyn/Queens (Bilingual in Hebrew or Yiddish)

The Care Manager provides person-centered care management services for individuals with intellectual and developmental disabilities, overseeing access to services, developing and implementing Life Plans, and advocating for members' needs. ACA/NY is a 501(c)(3) not-for-profit organization dedicated to meeting the needs of people with I/DD by providing comprehensive care management and coordination of services.

Requirements

• Bachelor's degree in a relevant field or equivalent experience
• Two years of relevant experience in care management or a related field
• Fluent understanding of community cultural traditions, norms, and practices of members and their families
• Excellent interpersonal, public speaking, and written communication skills
• Ability to work autonomously and in a team environment
• Demonstrated professionalism, respect, and ability to work in a team environment
