Care Manager - Brooklyn/Queens (Bilingual in Spanish)

The Care Manager provides person-centered care management services to individuals with intellectual and developmental disabilities, coordinating access to services, developing Life Plans, and advocating for members' needs.

Requirements

• Bachelor of Arts or Science degree with two years of relevant experience
• Fluent understanding of community cultural traditions, norms, and practices of Members and their families
• Excellent interpersonal, public speaking, and written communication skills
• Ability to work autonomously and demonstrate professionalism, respect, and ability to work in a team environment
• Work Environment: flexible work environment position with a blend of work from home, field work, and regional travel
