Job Description
Advantmed is seeking a skilled and compassionate Certified Nurse Practitioner to conduct in-home wellness risk adjustment assessments for Medicare members in Gaston County, North Carolina. This PRN role offers flexibility and opportunity to help value-based care of high-risk populations, focusing on proactive engagement and communication. The role involves comprehensive health assessments, pain management, cognitive assessments and collaboration with members' families.
Requirements
- NP License for the state you will be working in
- Valid unencumbered NP License
- 3+ years patient care experience (primary care/adult/geriatric preferred)
- Access to reliable transportation
