Job Description
Advantmed is seeking a highly skilled, compassionate Nurse Practitioner to conduct in-home wellness risk adjustment assessments for Medicare members and at-risk populations. The role involves thorough assessments of member health and well-being, providing individualized quality care, and assisting in closing quality gaps. This is a flexible part-time position offering opportunity for those balancing competing priorities.
Requirements
- Valid unencumbered NP License for the state of employment
- Ability to travel up to 55 miles radius
- Prior in-home risk assessment experience (preferred)
- 3 years patient care experience preferred (primary/adult/geriatric/EMR)
- Access to reliable transportation
- Strong ability to work independently
- Bilingual a plus
