Job Description
This role must be held by an RN. The Clinical Manager (RN) is responsible for managing patient discharges, developing home care plans, and serving as a liaison with hospitals and healthcare teams.
Requirements
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) licensure in the state of practice
- Valid Driver's License (DL) and must be at least 21 years of age or older
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
