Job Description
Advocates is seeking a Case Manager to provide documentation and maintenance of program participants’ records and plans, focusing on individual advocacy, empowerment, and community integration. The role involves providing services and support to individuals in areas like ADL, behavioral treatment plans, communication, and community integration. Advocates prioritizes a healthy work-life balance and offers professional development opportunities.
Requirements
- Provide case management, skills training, and support to individuals.
- Develop and implement ISP and intervention strategies.
- Assess and monitor individual behavior and data.
- Assist with obtaining services and entitlements.
- Foster community integration and support individual goal attainment.
- Participate in weekly staff meetings and clinical meetings.
