Job Description
Advocates is seeking a Direct Service Worker to support individuals experiencing mental health and/or substance use challenges within the Deaf Services Division. This entry-level role involves supervising program residents, providing ongoing support and guidance, and ensuring they achieve their goals through individualized plans and community integration. Advocates promotes work-life balance and offers opportunities for advancement.
Requirements
- Develop and document Comprehensive Assessments (CAs)
- Participate in the development of Individualized Action Plans (IAPs)
- Document daily in CBFS Notes
- Perform interventions with clients
- Communicate program participant’s progress and any changes in daily notes
- Provide linkage with other program staff and other service providers
- Monitor and document medication administration
- Ensure a clean, safe and home-like environment
- Ensure all paperwork, reports, trainings are up to date and completed
- Attend and actively participate in all training and maintain certification of required trainings (CPR/First Aid, SOLVE, MAP)
