Bereavement Coordinator - Alameda county

The Bereavement Coordinator will plan, implement, and maintain a bereavement program for families and caregivers within the American Hospice and Home Health Care Services Inc. They will provide grief counseling, support, and referral services, ensuring dignity and respect for all involved. This is a part-time position within the Alameda County hospice care network.

Requirements

• Coordinates the assessment and delivery of grief counseling needs and services.
• Plans for Bereavement Program development, expansion and refinement through annual evaluation of services.
• Provides bereavement support to hospice personnel coping with work related grief through counseling and referrals.
• Provides bereavement information and referral services to callers from the community.
• Assists as needed in evaluating and planning services to meet hospice personnel support needs.
• Actively participates in hospice activities.
