Job Description
AndHealth is seeking a Complex Care Clinical Pharmacist to deliver exceptional patient care and optimize medication therapy. This role involves providing patient-facing clinical services, educating patients, and collaborating with physicians and other healthcare professionals to improve patient outcomes. It emphasizes a collaborative and patient-centric approach.
Requirements
- Doctorate in Pharmacy (PharmD) or equivalent (e.g., Ph.D. in Pharmacy or equivalent)
- Unrestricted Registered Pharmacist license in Indiana
- Basic Life Support (BLS)/CPR Certification
- American Pharmacists Association and (APhA) Pharmacy-Based Immunization Delivery Certificate
- 3+ years of direct patient care experience in outpatient pharmacy preferred
- Medication Therapy Management experience strongly preferred
- PGY1 residency or equivalent work experience preferred
