Job Description
Angels of Care is seeking a Clinical Case Manager to coordinate patient and family care, provide support and guidance to care teams, and ensure compliance with regulations. The role involves conducting assessments, developing care plans, and mentoring staff, with a focus on pediatric home health. Responsibilities include managing caseloads, completing supervisory visits, and communicating updates to team members. This position requires traveling to the McKinney, TX area, plus office work in Sherman, TX.
Requirements
- Registered Nurse licensed with the applicable State Board of Nursing
- Graduate of an accredited nursing program
- CPR certification
- Current valid Driver’s License
- Proficiency with computer applications
- Strong interpersonal, organizational, and communication abilities
