Job Description
Work as a Clinical Case Manager, coordinating care and resources for children and their families. This role involves patient home visits, administrative tasks, and leadership of care teams.
Requirements
- Registered Nurse licensed with the applicable State Board of Nursing
- Graduate of an accredited nursing program
- CPR certification
- Current valid Driver’s License, good driving record, reliable transportation, and current auto liability insurance
- Proficiency with computer applications, including Microsoft Office and scheduling software
- Strong interpersonal, organizational, and communication abilities
