Case Manager

The Case Manager is responsible for assessing participants' physical and mental wellness, needs, preferences and abilities and using these to develop a tailored whole-person case plan in rural and tribal areas of Arizona. This includes performing healthcare assessments, coordinating care plans, addressing barriers to health, and providing support to participants in crisis situations.

Requirements

• Ability to pass a background check
• AZ Driver's License, reliable transportation, and ability to travel countywide and statewide
• Automobile Insurance in the amount of $100,000/$300,000
• Some evening and weekend hours as needed
