The ASCA Cares Program Coordinator is responsible for the daily oversight, growth, and success of the ASCA Cares program, which provides support to independent living residents through assessment, care plans, and personalized services.
Requirements
- Bachelor’s degree required in Social Work, Nursing, Public Health, Gerontology, Human Services, or a related field
- Minimum 3-5 years of care coordination, case management, or program development within senior living, healthcare, or community-based services
- Knowledge of senior services, care planning, and community resources
- Proficiency in billing practices, documentation standards, and regulatory compliance in senior care or healthcare settings
- Strong communication, organizational, and problem-solving skills
- Proficiency in Microsoft Teams (Office, Word, Excel)
