CARE COORDINATOR - Health Homes

Comprehensive Healthcare seeks a Care Coordinator to work with individuals who have chronic health conditions and are transitioning from inpatient stay or are high users of the healthcare system. The Care Coordinator will provide services and coordination to clients to ensure continuity of care across health and social service programs.

Requirements

• Bachelor's degree in social work, psychology, or related field
• Must be eligible for registration as Agency Affiliated Counselor
• Must have valid driver's license and vehicle liability insurance
