Case Manager

Axiom Care is seeking a case manager to provide temporary housing, recovery education, and employment assistance to justice-involved and substance-use individuals. This role focuses on the social determinants of health to help clients successfully reintegrate into the community and achieve independence. Case managers will maintain strict adherence to ethical standards and provide excellent client care, including documentation, referral coordination, and communication with stakeholders.

Requirements

• Proficient in computer applications and electronic medical record systems
• Proficient with Microsoft Office Suite
• Ability to communicate effectively with clients, clients supports and professionals
• Ability to make sound decisions in emergency situations
• Ability to define problems and draw valid conclusions
• Minimum one year of case management experience with the AHCCCS population, justice involved persons, chronically underhoused and the substance use treatment
