Substance Use Counselor

Compassionate Substance Abuse Counselor Needed! The Substance Abuse Counselor is responsible for supporting patients through medically assisted treatments and providing techniques to manage opioid addiction.

Requirements

• Conducts individual initial assessment and evaluation interviews for service eligibility determination
• Facilitates establishes and reviews with each patient the initial treatment plan
• Provides individual and group counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment
• Involves families and significant others in patient’s recovery process whenever possible
• Responds, as needed, to patient grievances and complaints
• Develops aftercare plans and discharge plans
