Mobile Crisis Case Coordinator - Madras

Seeking a Mobile Crisis Case Coordinator to work on a 2-person Mental Health Mobile Crisis team in Madras/Jefferson County. This full-time position involves crisis response, risk assessment, and crisis stabilization planning for adults and children in the County.

Requirements

• Provides crisis and risk assessment and intervention services
• Gathers information necessary to identify and assess risk factors for harm to self or others
• Provides assessment and recommendations to treatment providers and emergency departments
• Operates in a sometimes ambiguous and frequently changing work environment
• Respects client rights and responsibilities and demonstrates professional boundaries and ethics
• Documents and maintains electronic health record (EHR) and ancillary records
• Attends seminars, training, and other educational opportunities
• Collaborates effectively with other team members and community partners
• Completes comprehensive safety plans and follow-up with clients
• Assists in completing referrals to respite or other higher levels of care
• Acts as a role model to clients to inspire hope and share life experiences as appropriate
