Case Manager

Boca Recovery Center is seeking a Case Manager to provide comprehensive case management services to clients throughout their treatment journey. This role involves assisting with discharge planning, resource linkage, documentation, and advocacy, as well as coordinating community resources. The ideal candidate will be organized, empathetic, and knowledgeable in addiction recovery support. The company provides evidence-based clinical care in a supportive, structured environment.

Requirements

• Complete initial and ongoing case management assessments.
• Assist with scheduling medical, dental, and other ancillary appointments.
• Provide support for clients applying for food assistance or managing FMLA/unemployment paperwork.
• Help clients develop life skills including budgeting, communication, and personal care.
• Assist with job searches, resume writing, and interview preparation.
• Monitor phone calls when required and assist with employment-related needs.
• Coordinate discharge planning and aftercare services.
