Job Description
Spring Valley Hospice is seeking Registered Nurse Case Managers to provide direct care to patients and families in a hospice setting. The RN Case Managers will assess patients and families, administer medication, and collaborate with interdisciplinary team members. Requirements
- Complete initial and ongoing comprehensive assessments of patients and families
- Initiate patient plan of care and reevaluate as necessary
- Administer medication and treatment as prescribed by the physician
- Counsel patient and family in meeting nursing needs
- Complete and maintain accurate clinical notes
- Communicate with physician and participate in interdisciplinary group meetings
- Ensure compliance with legal and regulatory requirements
