Job Description
At-Heart Home Care & Hospice is seeking a passionate and dedicated RN Clinical Manager to oversee high-quality, patient-centered care in Virginia Beach, VA. Requirements
- Conduct quarterly quality improvement meetings
- Collaborate with branch director on improvement initiatives
- Review and analyze data to identify trends and performance issues
- Conduct case conferences to ensure oversight of care
- Document and review charts to ensure adherence to standards
- Develop and implement performance improvement action plans
