Job Description
Brilliant Corners is seeking a Case Manager (PSH Scattered Site - Temporary) to provide intensive case management services to homeless individuals. The role involves assisting clients to achieve and maintain health, mental health, and housing stability through client-centered and 'whatever it takes' services. The Case Manager will also assist with housing placement, tenant support, and community connections.
Requirements
- 1 year experience working with homeless population
- Bachelor’s Degree in Social Work, Psychology, Sociology, or related field OR at least 2 years experience providing direct mental health and/or intensive case management services
- Experience working with individuals with mental illness, chronic health issues, and/or substance use disorder
- Knowledge, Skills, and Abilities: Strong interpersonal and client engagement skills, excellent communication skills, flexibility, and willingness to travel
- Valid CA driver's license and personal insured vehicle
