Job Description
Calvary Hospital and Calvary @ Home, a 125-year veteran in palliative care, is seeking an Admission Nurse to visit patients referred for admission to their assisted living and inpatient facilities. The role involves assessing patients, educating them and their families, and coordinating care with the interdisciplinary team, with occasional travel on weekends. It is a compassionate role within a caring hospice team.
Requirements
- Current nursing license
- Current driver's license
- Graduate from an accredited nursing school
- 2 years of experience in acute care
- 1 year of hospice experience (preferred)
- Residency or BSN preferred
