RN Case Manager

Pikes Peak Hospice & Palliative Care is seeking a RN Case Manager to oversee and coordinate patient and family care in accordance with hospice philosophy and delivery of care.

Requirements

• Current Colorado RN license
• Current CPR certification
• Valid Colorado state driver’s license and proof of auto insurance
• 1-2 years experience in hospice/palliative care/home health, medical surgical, public health, oncology, acute care or home health
• Ability to lift/push/pull minimum of 35 lbs
