Job Description
CareHarmony is seeking a Remote, Full-Time Licensed Practical Nurse – LPN Nurse (LPN) (NLC) (LVN) to deliver value-based care management initiatives for their patients. The role focuses on providing patient education, medication management, chronic care coordination, and facilitating timely services.
