Job Description
CareHarmony is seeking a motivated and detail-oriented Licensed Practical Nurse (LPN) (LVN) to provide comprehensive patient care remotely. The role involves identifying and coordinating patient resources, providing education, managing medications, and assisting with their healthcare journey. This is a full-time, 100% remote position with no weekend work, and opportunities for career growth.
Requirements
- Manage patient census and close gaps in clinical and non-clinical care.
