Job Description
CareHarmony is seeking a Remote, Licensed Practical Nurse (LPN) to provide value-based care management to patients. The position requires a self-motivated individual with 3+ years of patient-facing experience and a dedication to impacting patient care. This role offers a fully remote position with a consistent schedule, career growth potential, and a supportive company culture.
Requirements
- Manage patient census and close gaps in clinical and non-clinical care.
- Identify and coordinate community resources for chronic care management patients.
- Provide patient education and health literacy on managing chronic conditions.
- Perform medication management and assist with timely service delivery.
- Resolve patient questions and create communication.
- Assist in appointment scheduling and referrals.
- Active Multi-State/Compact License (LPN) (NCL) (LVN) and Technical aptitude.
- Experience with Epic is a plus.
