Job Description
CareHarmony is seeking a Remote LPN (LVN) to provide value-based care management initiatives for patients. The role focuses on identifying resources, coordinating chronic care management needs, educating patients, and managing medication. This is a full-time, full-time position with a focus on self-motivation and detail-orientation, offering a remote work environment with a fast-paced schedule and potential for career advancement.
Requirements
- Manage patient census and resolve gaps in clinical/non-clinical care.
- Identify and coordinate community resources for patients.
- Provide patient education and health literacy.
- Perform medication management, including refills.
- Manage referrals and appointment scheduling.
- Active Multi-State/Compact License (LPN) (NCL) (LVN).
