Job Description
CareHarmony is seeking a Remote, Licensed Practical Nurse (LPN) (LVN) to provide value-based care management initiatives for patients. The role involves managing patient populations, coordinating resources, and providing patient education. This is a fully remote position with a strong focus on consistent hours and career growth opportunities.
Requirements
- Active Multi-State/Compact License (LPN) (NLC) (LVN)
- Technical aptitude – Microsoft Office Suite
- Excellent written and verbal communication skills
- Plusses: Epic Experience, Bilingual
- Remote Requirements: Must have active high-speed Wi-Fi, a home office/HIPAA-compliant workspace, and the ability to understand, process, and take thorough notes in real-time, often by telephone.
