Licensed Clinical Social Worker

The Licensed Clinical Social Worker assists patients and their families understand a particular illness, work through the emotions of a diagnosis, and provide therapeutic consultation in order to improve psychosocial functioning within a primary care setting. Works in collaboration with CPH clinical staff (doctors, nurses, and allied health professionals).

Requirements

• Coordinates all psychosocial services under the general direction of the Clinical Psychologist/Psychiatrist
• Establishes and maintains positive, effective communication and interpersonal relationships with patients, associates, managers, and physicians to facilitate continuity of client’s medical and psychosocial plan of care
• Resolves patient concerns and complaints at the lowest level
• Participates in staff meetings and team huddles to maintain associates are up to date with general changes
• Responsible for assessing problems and determining appropriate types and methods of treatment and implementing intervention methodologies
• Assist in the assessment, planning, arranging, and implementing of psychosocial treatment plans
• Counsel patients and interpret professional and lay groups the special psychosocial needs of patients by identifying psychosocial disorders
• Coordinate with clinical, health education, community health and other staff members in the provision of services to patients
• Provide referrals for services needed
• Coordinate with other organizational entities involved in psychosocial and mental health services
• Provide in-service presentations to staff as needed or required
• Perform other related duties as assigned
