Job Description
The Licensed Clinical Social Worker promotes efficient coordination of patient care through collaborative practice within the current health care team’s facilities, providing social work case management functions and behavioral health and addiction counseling to patients and families.
Requirements
- Master’s Degree in Social Work from a CSWE accredited program
- Masters Social Worker License (LMSW) with valid State of Michigan license
- Michigan Certification Board of Addiction Professionals (MCBAP) required or willingness to obtain
- Ability to maintain compliance with applicable laws such as patient confidentiality, HIPPPA, Americans with Disabilities Act, abuse reporting, [principles of consent and advanced medical directives]
- Knowledge of HIPPA and 42CFR as use of Protected Health Information (PHI) is an integral part of the position and privacy of individuals must be protected in accordance with federal and state law
- Knowledgeable of community resources
- Flexible, with the ability to work independently and with minimal guidance
- Strong oral and written communication skills
- Excellent interpersonal and customer service skills, to effectively and tactfully deal with patients and staff
- General computer proficiency
