Patient Navigator (58202)

Circle the City overview: Circle the City (CTC) is a nonprofit community health organization with a mission to create and deliver innovative healthcare solutions that compassionately address the needs of men, women and children facing homelessness. The Patient Navigator works primarily within the integrated health care team with the goal of improving access to care and patient outcomes.

Requirements

• Enable non-clinical services that facilitate access to timely and appropriate care, such as referrals to transportation, case management, and education.
• Assist patients with completing brief applications and registration forms.
• Assess for eligibility of DES benefits and coordinate benefits enrollment through follow-up with uninsured patients to ensure timely referral to DES and/or other community eligibility service providers.
