We are seeking a dedicated and compassionate Forensic Case Manager to join our team. This role is responsible for providing case management and support to justice-involved individuals with mental health and/or co-occurring disorders.
Requirements
- Conduct assessments to identify client needs, strengths, and risks in collaboration with the treatment team
- Develop and implement individualized case plans that address mental health, substance use, housing, employment, and other recovery-related goals
- Monitor client progress and compliance with court-ordered conditions and treatment plans
- Advocate on behalf of clients with courts, probation/parole officers, treatment providers, and community agencies
- Facilitate connections to community-based services, including housing, vocational programs, education, and healthcare
- Provide crisis intervention and support as needed
- Maintain accurate and timely documentation of client interactions, progress, and outcomes
- Participate in multidisciplinary team meetings to coordinate care and support for clients
- Educate clients about their rights, responsibilities, and available resources
- Support clients in building skills for independent living, self-advocacy, and positive social connections
- Ensure compliance with all organizational policies, confidentiality requirements, and applicable regulations
- Participate in quality improvement initiatives, staff training, and professional development
