Job Description
At Community Hospice & Health Services and its affiliates, Community Hospice & Health Services Foundation, and CHI Management, Inc. Requirements
- Assists with environmental services
- Assists the patient with personal care
- Communicate with other hospice team members
- Submits visit notes and timesheets
- Cooperates with regulatory requirements
- Communicates expected arrival times
- Attends regularly scheduled care conferences
- Attends death events and provide appropriate support
- Participation in Community Hospice & Health Services orientation of students
- Participates in in-services, workshops, and clinical meetings
- Follows all Community Hospice & Health Services policies and procedures
- Maintains a professional appearance of self and working area
- Exhibits appropriate behavior with guests and clients

- Valid California Auto Insurance must be maintained at all times
