Job Description
The Residential Case Manager provides case management and supportive services to residents in CHS Residential programs. This role involves working with residents to achieve their goals, collaborating with support staff, managing crisis situations, and completing required documentation. The RCM is cross-trained and works across all residential sites as needed, focusing on housing as the immediate need, without requiring abstinence from harmful behavior.
Requirements
- Develop a professional and therapeutic relationship with residents.
- Maintain and model appropriate personal boundaries.
- Provide direct case management regarding goals, budgets, and personal needs.
- Collaborate with Housing and Employment Coordinators to develop individualized goals.
- Assist with participant enrollment in life skills workshops.
- Complete required written documentation.
