Job Description
CLASS is seeking a Licensed Practical Nurse (LPN) to provide high quality, person-centered care and support to individuals with disabilities and seniors throughout Allegheny County.
Requirements
- Provide guidance and direction to DSPs and RHMs related to medical concerns and needs
- Act as a hospital, Skilled Nursing Facilities (SNF), and overall health care liaison
- Coordinate hospitalization discharges, including completion of in-person assessments
- Support individuals in achieving health-related goals and advocate for their medical needs
- Complete Health Risk Screening Tool (HRST) clinical reviews
- Participate as a member of the Individual Service Plan (ISP) team as needed
- Participate in the completion of the intake assessment on all new admissions
- Ensure compliance with regulations set forth by the state, federal, ODP, and OLTL
