Job Description
Provide Peer Mentor services to persons assigned to caseload, assisting with daily living skills, relationship building, and symptom self-management through self-disclosure of personal recovery experience.
Requirements
- Provide Peer Mentor services to persons assigned to caseload
- Mentoring through self-disclosure of personal recovery experience
- Assist in planning and implementing recovery group activities
- Make all contacts as indicated in tx plan
- Weekly contact sheets turned in at the end of the workweek
- Progress notes are completed within 24 hours of service
- Regular contact with the treatment team
