Job Description
Transition Coordinator develops collaborative and comprehensive discharge plans for clients of Inpatient & Residential programs. Responsibilities include crisis evaluation/intervention, financial assessment, and supportive individual counseling.
Requirements
- Bachelor's degree in social work, behavioral science, or related field
- Must be Washington State Agency Affiliated Counselor
- Driver's license and vehicle liability insurance
