The Case Manager will assess, coordinate, and implement a medical plan of care for children in homeless shelters, developing a program to oversee their healthcare and measuring its impact.
Requirements
- Associate degree in nursing
- At least three years of experience in case management, care coordination, utilization review, patient intake, discharge planning, and troubleshooting fund resources
- Computer literacy
- Effective organizational, interpersonal, written, and oral communication skills
- Familiarity with community resources and charitable organizations
- Ability to adapt to changing healthcare environments
- Experience using electronic medical records
- Current RN license from Texas Board of Nursing
- Basic Life Support (BLS) certification
- Case Manager Certification (CCM) preferred
- Bilingualism is preferred or may be required
