Job Description
The Behavioral Health Practitioner will provide coaching and coordination of care for patients with complex needs in a primary care or telehealth setting. They will assess patients to determine appropriate service and collaborate with various healthcare professionals to create tailored care plans and track progress. The role emphasizes patient well-being, behavioral health promotion, and integration into community resources.
Requirements
- Valid and current LCSW, LMHT, LPCC, LMFT, LCADAC, LICSW, or LMSW CA licensure
- 3-5 years’ care management and/or managed care experience
- Bilingual Spanish/English
- 2-5 years of leadership experience in a care management/managed care setting (preferred)
