Job Description
The Behavioral Health Practitioner (BHP) will join a multidisciplinary healthcare team in a primary care/telephonic setting. They will focus on coaching and coordination of care for patients needing navigation and addressing their care needs and follow-up. Responsibilities include observation, assessment, and therapeutic intervention, and utilizing available services. The role involves collaboration with APPs and other providers.
Requirements
- Valid and current LCSW, LMHT, LPCC, LMFT, LCADAC, LICSW or LMSW CA licensure
- 3-5 years’ care management and/or managed care experience in one of the following settings
- Bilingual Spanish/English
