Care Coordinator

The Care Coordinator (CC) facilitates patient continuity of care and preventive care within the care management team, collaborating with patients, family, physicians, nurses, and other healthcare team members to serve needs and expedite cost-effective care.

Requirements

• Support individualized patient care plans
• Address patient risk factors and/or obstacles to care
• Facilitate engagement between PCP and patient
• Maintain predetermined patient caseload
