Job Description
The Community Health Coordinator (CHC) is responsible for helping members and their families to navigate and access community services, and other resources, to support Social Drivers of Health including overall and maternal health, housing, transportation and early childhood education.
Requirements
- Valid California Driver’s License
- High school graduate or equivalent
- Associates Degree with areas of focus in social work, public health, behavioral health, administration or related field
- Basic computer skills
- Written and oral fluency in English
- Spanish fluency may be required for some locations
- Experience working in a multi-cultural and community-based setting for at least 1 to 2 years
