The Community Health Worker is responsible for helping patients and their families navigate and access community services, resources, and adopt healthy behaviors. The CHW supports providers and Case Managers through an integrated approach to care management and community outreach.
Requirements
- Educating members about ECM services and assisting them with enrollment
- Supporting individuals and families as they navigate the healthcare system and transition to self-care and healthcare management
- Providing ongoing follow-up, basic motivational interviewing, and goal setting with patients/families
- Helping bridge conversations with members and removing barriers to accessing health and social services
- Collaborating with the full care team to create individualized care plans
- Assisting members in accessing health-related services and community resources
- Documenting interactions with members and on behalf of members in medical records
- Following up with patients via phone calls, home visits, and visits to other settings
- Providing referrals for services to community agencies as appropriate
- Helping patients connect with transportation resources and providing appointment reminders
