Work with families to assess their strengths and support them in setting and meeting individual and family goals. Guide families toward healthy self-reliance and positive relationships.

Requirements

• Recruit and enroll eligible families
• Develop a rapport and provide service through ongoing interactions
• Complete and maintain Family Strengths Assessments and Family Partnership Agreements
• Support families in accomplishing their identified goals
• Practice real-time data entry to ensure program integrity
• Partner with local community resources and agencies
• Advocate for families
• Assist families in finding medical and dental homes
• Guide families to utilize available services
• Team with teachers to ensure effective communications
