LPN/Resident Care Coordinator

We are seeking an experienced and dependable LPN / Resident Care Coordinator (RCC) to join our clinical leadership team. This role is hands-on and requires someone who is comfortable balancing resident care coordination, medication oversight, and floor support when needed.

Requirements

• Current Colorado LPN license in good standing
• Experience in Assisted Living, Memory Care, or Long-Term Care preferred
• Strong medication management and clinical judgment skills
• Ability and willingness to assist with hands-on resident care
• Experience supervising or leading care staff strongly preferred
• Strong documentation and communication skills
• Ability to remain calm, organized, and decisive in a fast-paced environment
